Smart Health Intake

Before creating an account, you'll complete a guided intake questionnaire tailored to your treatment area — weight loss, men's health, hair loss, or general care. This gives your physician a clinical picture before you even meet.

Create Your Secure Account

After intake, you're directed to registration. Your intake data is already linked — just set up your login credentials and secure your account.

  • Confirm your name, date of birth, and contact details
  • Create a strong password (we enforce NIST security standards)
  • Set up two-factor authentication (2FA) using an authenticator app like Google Authenticator or Authy — required for all accounts
  • Review and sign the HIPAA Notice of Privacy Practices and Terms of Service

Two-factor authentication is mandatory. It protects your health records from unauthorized access — even if your password is compromised.

Identity Verification (IAL2)

Before any prescription can be written, we verify that you are who you say you are. This is called IAL2 — Identity Assurance Level 2 — the federal standard used by healthcare platforms for safe prescribing.

  • Step 1 — Government ID: Upload a photo of your government-issued ID (driver's license, state ID, or passport) — front and back
  • Step 2 — Live Face Scan: Complete a quick 3-angle face scan (front, turn left, turn right) directly in your browser — no app needed. Takes about 60 seconds.
  • The face scan is captured live and watermarked with your email and timestamp — it cannot be a photo of a photo
  • Your ID images are encrypted with military-grade encryption on our servers and never shared with third parties

What is IAL2 and why do we require it?

IAL2 (Identity Assurance Level 2) is the NIST standard that verifies a person's identity is real and belongs to them. We require it because prescribing medications to the wrong person isn't just a legal issue — it's a patient safety issue.
