Tadalafil is a phosphodiesterase type 5 (PDE5) inhibitor in the same class as sildenafil (Viagra), but with a much longer duration of action. It is the active ingredient in Cialis® and has been available generically since 2018. Tadalafil is FDA-approved for three indications: erectile dysfunction, benign prostatic hyperplasia (BPH) symptoms, and (at different doses) pulmonary arterial hypertension.
Its defining feature is duration. A single as-needed dose (10–20 mg) remains effective for up to 36 hours — nearly ten times longer than sildenafil. This is why it is sometimes called "the weekend pill." Alternatively, tadalafil can be taken as a low daily dose (2.5–5 mg) that produces continuous coverage without ever having to time a pill around activity.
