Tadalafil is a phosphodiesterase type 5 (PDE5) inhibitor in the same class as sildenafil (Viagra), but with a much longer duration of action. It is the active ingredient in Cialis® and has been available generically since 2018. Tadalafil is FDA-approved for three indications: erectile dysfunction, benign prostatic hyperplasia (BPH) symptoms, and (at different doses) pulmonary arterial hypertension.
Its defining feature is duration. A single as-needed dose (10–20 mg) remains effective for up to 36 hours — nearly ten times longer than sildenafil. This is why it is sometimes called "the weekend pill." Alternatively, tadalafil can be taken as a low daily dose (2.5–5 mg) that produces continuous coverage without ever having to time a pill around activity.
Both strategies work. The right choice depends on how often you are sexually active and whether you have BPH symptoms:
Most patients who try both prefer daily dosing. Removing the mental load of timing a dose is, for many men, the single biggest quality-of-life improvement a PDE5 inhibitor offers.
Tadalafil 5 mg daily is the only PDE5 inhibitor FDA-approved for both erectile dysfunction and BPH. Men with an enlarged prostate often experience bothersome urinary symptoms: frequent urination (especially at night), weak stream, sense of incomplete emptying, and urgency. Tadalafil relaxes smooth muscle in the prostate and bladder neck, improving these symptoms in many patients.

Most common side effects are mild and often diminish with continued use: headache, back pain, muscle aches, nasal congestion, indigestion, and flushing. Daily low-dose tadalafil typically produces fewer side effects than the higher as-needed doses, since peak plasma concentration is lower.
Seek emergency care for any of the following: erection lasting more than 4 hours (priapism), sudden vision loss, sudden hearing loss, or chest pain during sexual activity.

Moderate alcohol is generally safe with tadalafil. Both tadalafil and alcohol are vasodilators; heavy alcohol intake with tadalafil can increase risk of dizziness, headache, and low blood pressure. Stick to moderate consumption.
No. Do not combine PDE5 inhibitors. If switching from one to the other, wait at least 24 hours (sildenafil to tadalafil) or 48 hours (tadalafil to sildenafil) before taking the other.
