What Is Wegovy HD?

Wegovy HD (semaglutide 7.2mg) is a higher-dose injectable formulation of semaglutide, approved by the FDA on March 19, 2026. It is the same molecule as standard Wegovy — just at three times the currently approved injectable dose. It comes in a single-dose pen (7.2mg/0.75mL) and is injected once weekly.

The STEP UP Trial Results

20.7% mean weight loss at the studied endpoint (efficacy estimand — patients who stayed on treatment). Approximately 1 in 3 patients achieved ≥25% weight loss.

Who Qualifies

Wegovy HD is approved as a step-up dose only — not a starting dose. You must have tolerated semaglutide 2.4mg for at least 4 weeks before escalating.

The Important Side Effect: Dysesthesia (Skin Sensitivity)

Standard Wegovy side effects also occur with Wegovy HD, but there is one side effect significantly more common at the higher dose:

Dysesthesia (altered skin sensation):
  • Wegovy HD 7.2mg: 22% of patients
  • Standard Wegovy 2.4mg: 6% of patients
  • Placebo: 0%

This is a meaningful jump. The sensation is generally described as skin hypersensitivity — touch, temperature, or fabric feeling more sensitive. It is not dangerous but can be distressing. Patients must be counseled before starting.
