After your visit

After Your Visit: Turn Your Care Plan Into Next Steps

A visit is only useful if you know what to do afterward. Before closing your visit summary, separate the plan into medicines, tests, things to do at home, and follow-up. You do not have to remember everything from the conversation.

What to do next

  1. Read the instructions once without multitasking. Mark anything that differs from what you understood during the visit.
  2. For each medicine, confirm what changed and which pharmacy will fill it. Do not guess whether an older prescription should continue.
  3. For each test or referral, write down who schedules it, where it happens, and who will discuss the result.
  4. Put the follow-up date on your calendar. If the plan says to return only if symptoms persist, ask what time frame that means.

When to contact your care team

Contact the care team if cost, transportation, language, side effects, or the instructions themselves make the plan difficult. A missing result or an unanswered message does not mean everything is normal. For an emergency, use emergency care rather than waiting for a portal reply.

Put it into practice

Try explaining the plan back in your own words: “Today I will ____. I will arrange ____ by ____. I will contact you if ____.” This is a way to find unclear instructions, not a test of your memory.

Notes for your next visit

Write these in your own notes or on a printed copy. Bring them to your follow-up.

  • The question I still need answered:
  • Who I contact if I cannot complete the plan:
